Intake and Release Forms

You may print the Release of Information form from here, using the button above. All the other intake forms are combined into one pdf document, which we will send you via Spruce, our scheduling app. You’ll need to bring all the completed forms with you to your first appointment.
If your first appointment is virtual, you may return the forms as photos or scan and email.
